This form is intended for activating H2O Wireless service with a new phone number for use strictly within the United States. All calls, texts, and data must originate in the U.S. and are subject to H2O Wireless’ coverage map.

By submitting this form, you agree to receive a new phone number assigned based on the general geographic location of the area code you provide. Activations cannot be scheduled for a future date, and once your plan is activated, no cancellations or refunds will be accepted.

These plans are intended for new H2O Wireless customers only. The SIM card must remain in the AT&T VoLTE-compatible device associated with the IMEI submitted during activation. If the SIM is moved to another device, H2O Wireless may disconnect your service, and no refund will be issued.

You may submit up to five SIMs per form, and each IMEI must be unique and not previously used on the H2O Wireless network. Activation is typically completed within a few hours. By submitting this request, you agree to all terms and conditions of JZN Market and H2O Wireless.
