This form is intended for activating H2O Wireless service and porting in to H2O Wireless an existing U.S. based phone number for use strictly within the United States. All calls, texts, and data must originate in the U.S. and are subject to H2O Wireless’ coverage map.
By submitting this form, you agree to port-in your existing U.S. based phone number to H2O Wireless. Port-Ins / activations cannot be scheduled for a future date, and once your plan is activated, no cancellations or refunds will be accepted.
These plans are intended for new H2O Wireless customers only. The SIM card must remain in the AT&T VoLTE-compatible device associated with the IMEI submitted during activation. If the SIM is moved to another device, H2O Wireless may disconnect your service, and no refund will be issued.
Port-In / activation is typically completed within a few hours.
